Our Practice
While many of our patients are referred by other medical doctors, we are happy to see patients without a referral. Our new patient paperwork may be accessed through the “patient resources” link on our website or you may arrive 15 minutes prior to your appointment time to fill out these forms in our office. We are happy to file all insurance paperwork on your behalf but please be sure to bring your drivers license and insurance card to your visit. All copays are due at the time of service. We will fill out all FMLA, disability, and other paperwork for a nominal fee.
Each patient will receive a thorough examination in our office prior to scheduling any surgical procedure. Only minor procedures which can be performed in the office may be done on the day of initial evaluation. After discussion and examination, we will determine if any further testing is needed or if procedures should be scheduled. Our staff will ensure that testing and surgery scheduling are accomplished in a timely fashion convenient for each patient.
